Support Services acts as a “unifier” in the mental health care system by linking young people and adults to services specific to their needs. Support is tailor-made, matching community programs to each individual.  By integrating and personalizing services, care is intended to provide a solid building block  for an individual’s development towards self-reliance and self-improvement.

Adult Services

Adult Case Managers provide the bridge between the individual and the adult mental health support systems in the community. The Case Managers monitor service delivery and respond to changes in a person’s needs.  Direct care is also provided through teaching, community integration, supportive counseling and crisis intervention.

Child & Adolescent Services

Youth Case Managers help children, adolescents and families identify and benefit from services at schools, clinics, health care centers or social services.  The Case Managers support by providing short term and informal counseling, referral, and transportation.

Benefits Management

This service assists with money management and benefits. Representative payee services are available to those receiving Social Security.  Individuals are also assisted in applying for benefits in Social Security appeals, hearings, re- and pre-determinations and reporting changes.

Dual Recovery

Dual Recovery Case Managers provide systems integration by improving client access to comprehensive services and continuity of care, facilitate sharing of information, and collaboration among our agencies to improve outcomes for the homeless, and provide linkage with the mental health and substance abuse services, housing, health-care and income supports and entitlements.  The target population includes persons 18 years of age and older who are homeless and have a serious mental illness with an emphasis on those who have alcohol and/or drug problems and are involved in the justice system.
